Yes, currently our services are appointment only so that our clients are treated quickly and efficiently.

Bring your license and the filled out DOT forms as well as any required vision or hearing correction (glasses or hearing aids). If you have diabetes, bring labs within the past 6 months showing a hemaglobin A1c of less than 10%. If you use insulin, have a copy of the FMCSA diabetes form filled out by your clinician. If you have sleep apnea on CPAP, bring the compliance report from the machine. If you are on controlled substances, bring a letter from the prescriber stating the medication does not interfere with driving.

Fill out and bring the History & Physical form specific for your school. Bring an ID card with your picture on it. if you are under 18 y/o, bring an adult guardian. If you wear glasses, bring them as well.

Most physicals take only 15-20 minutes. Allow more time if you are filling out forms in the office.

Yes, we do drug screens and breath alcohol testing for commercial and private clients who have contracted for those services.

It aapears the California DMV is now requiring proof of citizenship for CDL renewal. This is preventing us from uploading the DOT physical results. We will resume uploading if this problem gets resolved.
